My name is Jaime Puzauskas and this is my 17th year with the Upper Perkiomen School District. I am very excited to be your child's 3rd grade teacher this year.

I graduated from Elizabethtown College with my certifications in both Elementary and Special Education. I completed my Master's Degree at Kutztown University and received my certification as a Reading Specialist. I spent my first 6 years at Upper Perk as a learning support teacher in 4th and 5th grade, followed by 9 years in kindergarten, and now I am enjoying teaching 3rd grade students.

I live nearby in the Boyertown school district with my husband and two children. My son, Carter, is a 5th grader and my daughter, Lila, is a 2nd grader. We stay very busy with both kids' sports and activities. In my downtime, I enjoy reading, hiking, puzzles, playing word games and being outdoors.

Supply list for 3rd grade students:

• 4 2-pocket folders (different colors for each subject)

• 1 pencil box or zipper pouch

• 1 - 2 dozen pencils (to be used throughout the year- please keep extras at home)

• 1 package of 12 - 24 colored pencils

• 2- - 4 highlighters (various colors)

• 2 - 4 dry erase markers

• 1 dry erase eraser (this can be an old sock)

• 2 small erasers

• scissors 

• 1 large or 2 small glue sticks

• Earbuds/headphones  (Corded Only - No Bluetooth)

Donations of tissues and cleaning wipes are helpful throughout the year. There are sometimes occasional requests made for special projects so if you're able to donate when those requests come home, it is always appreciated!  If you need assistance with supplies, please let me know.

The easiest way to order is placing your order through Scholastic Book Club orders online.

Here’s how it works:

• SIGN UP at www.scholastic.com/bookclubs. On the parent page, register by clicking the “Don’t have a user name or password?” link. When prompted, enter the one-time Class Activation Code shown below. This unique code ensures that your order is sent to me.

• SELECT the books you’d like to order.
• Pay for your order online using a credit card and your child’s books will be delivered directly to my classroom when I submit our classroom order.

You can still order using the form from the printed flyer. If you choose this method, please send your CHECK (made out to Scholastic Book Club) and order in a sealed envelope to school in your child’s folder. Please do NOT send cash.
