2021-22 Instructional Model Questionnaire

We are thrilled to have so many students back in-person each school day and that we are able to continue to support our students and families in the UPOLA/Virtual models for the remainder of the 2020-21 School Year. We have already started to plan ahead for next school year and we need some valuable information from you to help us continue in our planning process.  We need information not only from our current UPSD in-person and virtual families but also from our current Cyber Charter School, In-Person Charter School, Private/Parochial School and Homeschooling families as well. We need our entire school community to participate and support us in our planning process for a successful 2021-22 school year.  As we discuss the 2021-22 School Year, our hope and current plan is that we will return all UPSD students who wish to learn in person to full time, face to face instruction, five days per week. However, since we do not know what the future holds in regards to the pandemic, we want to be as prepared as possible.

Therefore, we are asking each Upper Perkiomen family to complete a questionnaire to gather preliminary information about next school year.  We are asking that you complete one questionnaire for each school age child (current students in grades K-11) and choose the preference for your child’s instructional model for the 2021-22 School Year. Your choices are Face to Face, in-person instruction five days per week OR a virtual model that will differ between Elementary (Grades 1 – 5) and Secondary (Grades 6 – 12). All incoming Kindergarten students will learn in person five days per week, as there will not be a virtual program for Kindergarten. Be sure to read the choices on the questionnaire carefully because the virtual options do vary by grade level, and the virtual options in some grade levels will look different than they do currently.  We are asking that you complete one questionnaire per student by Sunday May 2, 2021.

Please note: Completion of this questionnaire is only to gather preliminary data and is non-binding. All current UPSD families will be asked to complete a declaration at some point this summer and additional information will be sent out to our Cyber Charter School, In-Person Charter School, Private/Parochial and Homeschooling families as well ahead of next school year.  We understand that it is very early to be making concrete decisions about next school year, so we ask you to answer these questions to the best of your ability at this point in time.  We are asking for your input in order to help us make decisions in real-time due to the dynamic nature of the pandemic response.

The questionnaire links are below.  Please complete one questionnaire for each child currently in grades K-11 by Sunday May 2, 2021. There are four (4) separate questionnaire links below for current UPSD students by next year’s grade level (Grades 1-3, 4-5, 6-8 and 9-12) and two (2) additional questionnaire links below for our Cyber Charter/ In-Person Charter School students and for our Private/Parochial and Homeschooling students.  Please choose the correct questionnaire link for your student(s).  Thank you in advance for your cooperation and participation as we prepare for next school year.
