Step 1: Requirements Prior To Transition Meeting

Please click on and complete the following 3 requirements to fulfill step 1 . This information will be directly sent to the transition team at Upper Perkiomen to review in order to prepare for your meeting.

Requirement #1 - Student Registration

New Families:

Families that are new to the district must create a Temporary Account for New Student Enrollment. Once an account has been created, an email will be sent to you with a link to our online application. Please complete and submit the application in its entirety. Once submitted, the school district will review the application and contact you to finish the enrollment process. Please note the application MUST be completed by the parent or legal guardian.

Returning Families:

For CURRENT UPSD families who already have a Skyward parent account, please enroll your new student by logging into your Skyward account in Family Access.

Please note: the following documents are required for student to be enrolled in the UPSD. You can start the application at anytime without having documents, however, you will not be able to submit it until they are completed.

You must submit the following items to complete your registration:

Original Birth Certificate
Proof of Residence (Deed/Settlement Statement or Current Signed Lease Agreement)
Driver's License or Photo ID
Current Paid Utility Bill
Proof of the following completed immunizations:
- Four (4) DPT shots (including one dose administered after the fourth birthday)
- Three (3) doses of oral polio vaccine or four (4) doses of inactivated polio vaccine
- Two (2) doses of measles, mumps, and rubella vaccine (the first dose administered AFTER the first birthday)
- Three (3) doses of hepatitis B vaccine
- Two (2) doses of Varicella (chicken pox) vaccine or history of disease
Other required documents (if applicable)
- Most recent IEP/GIEP/504 Plan
- Court Ordered Custody or other legal documents

Please complete this brief parent input form so that we can learn more about your child prior to our transition meeting. The information may also be used in combination with the Parent/Guardian Questionnaire as part of the Comprehensive Evaluation Report to help make recommendations for your child’s educational program upon entrance to kindergarten. This information will be treated as confidential.

I have completed requirements 1-3, now what?

Preschool Children Receiving Early Intervention Services who will be entering Kindergarten:

If the child is receiving Early Intervention Services through the Montgomery County Intermediate Unit (MCIU), the MCIU will provide information to the parent(s)/guardian(s) concerning the transition process and the need for registration with Upper Perkiomen School District (UPSD). The MCIU will schedule a Transition Meeting with the parent(s)/guardian(s), an MCIU Representative, and the UPSD School Age Transition Team Representative. After residency has been satisfied, UPSD will schedule an Intake Meeting for parent(s)/guardian(s). At the Intake Meeting, the child's records will be reviewed, and the reevaluation process will be discussed. Upon approval from the parent(s)/guardians, UPSD will conduct a Reevaluation. Following the Reevaluation, educational programming will be determined.
