Required Paperwork for New Employees and Contractors

If you are offered a position as an employee or contractor, please be advised the following paperwork will be required before you are able to begin working.

Act 34: PA State Police Clearance (to be updated every 5 years)
- You can apply online and you will receive results immediately.
- Print and retain the receipt containing the control number.
- This clearance needs to be dated within one year of start date.

Act 151: Child Abuse Clearance (to be updated every 5 years)
- Create an account on the Child Welfare Portal or login if you have already done so.
- Read all information, create a Keystone ID (unless you already have one assigned) and complete all requested information.
- You will receive an email confirmation and you are able to view the status at anytime by logging in.
- This clearance needs to be dated within one year of proposed start date.

Act 114: FBI Fingerprint (to be updated every 5 years)
- Enter your service code (1KG6XN)
- Complete registration process
- Select a location and appointment time
  - As of 11/29/17, the closest location to UPSD is the following IdentoGO storefront:
  292 W Ridge Pike
- Bldg C Ste 101 Limerick, PA 19468
- Print the pre-enrollment form to take to your appointment with a check, money order or credit card

School Health Form (includes physical and TB test)
- Physical must be dated within one year of start date and be completed on the School Personnel Health Record Form provided at the link above.
- TB test results must be a maximum of three months prior to employment.

ACT 24: Arrest/Conviction Report and Certification
- This form needs to be filled out and signed (the employer retains the original of this form).
- By signing, you are stating that the information provided on this form is correct and complete.

Act 168 Form: Sexual Misconduct/Abuse Disclosure Release Form (for new employees and contractors only)
- One of these forms needs to be filled out and supplied for the following:
  - Your current employer
  - Each past employer in which you had direct contact with children

All current employees/contractors are required to update their clearances on file every 5 years (60 months).

As an employee, you can log into your Employee Access Center (Skyward) and go to Employee Information > Personal Information > Custom Forms > Clearances to view all clearances dates and when they expire.

Volunteers

Required Paperwork for Volunteers

Volunteers play a vital role in supporting the staff, students, and programs of our schools in the Upper Perkiomen School District (UPSD). Thank you for your interest in volunteering at UPSD.

You are considered a volunteer if you are coming in to any school building for more than the purposes of a one time presentation under a teacher's supervision or dropping something off in the main office. Volunteer's interact with students and move beyond the main office into the halls and participate in the daily activities of the students.

On October 22, 2014, Act 153 of 2014 was signed into law. This new law changed background check requirements for employees, contractors and volunteers. This law requires action on your part in order to remain or become a volunteer at UPSD. All employees, contractors and volunteers are required to obtain new clearances (child abuse history, state and federal criminal history checks) every 5 years (60 months).

If you are interested in becoming a volunteer, please be advised the following paperwork will be required before you are able to begin volunteering in the District.

Once the paperwork listed below has been completed please do the following:

Retain copies for your records

Send copies to the UPSD Human Resources Office through one of the following methods:

1. Scan and email to uphr@upsd.org (we do not need to see original forms as they can be verified online)

2. Mail copies to 2229 E. Buck Road, Pennsburg, PA 18073 Attention: HR

3. Stop in our office at the address above to drop off copies

Once all appropriate paperwork has been received and verified by Human Resources your name will be added to a list of active volunteers. This list is updated and provided to all school's central office staff who can verify that you are eligible to volunteer in any UPSD building.

The following must be received prior to being cleared to volunteer:

Substituting at the Upper Perkiomen School District

The Upper Perkiomen School District has partnered with an educational staffing company, ESS, Teacher Services and Educational Staffing Agency, to supply substitute teachers for the District. Student Interns & Student Teachers

Student Teacher/Internship Procedures

College/University/Student contacts Human Resources Director to inquire about a placement and completes the Student Teacher/Internship Request Form (see below under the “Student Teacher/Internship Required Paperwork” section)
Completed request forms must be submitted to the Human Resources Department by July 1 for a fall semester placement and November 1 for a spring semester placement.
Pre-arrangements and guarantees for specific teachers, classrooms, or schools should NOT be made prior to completion of the forms. Student teachers/interns must be willing to work in any school/classroom that is available for student teachers/interns and flexibility of location/classroom to balance out the student teachers/interns across the district.
The building principal reviews the request and selects a teacher mentor, if approved.
Student coordinates with college or university and UPSD Human Resources to confirm placement and submit required documents such as: clearances and TB Test Results.
UPSD Human Resources reviews and approves credentials.
College/University confirms placement dates with UPSD Human Resources.
UPSD Human Resources notifies the building principal, teacher mentor, and student of final placement.

Requests for placement will be taken on a first come, first served basis. There may be limits placed on the number of placements per grade level and building to maintain consistency in programming.

These procedures apply to all Pre-Service, Student Teaching, Internships, and Clinical Assignments within the Upper Perkiomen School District. Any questions regarding the above listed procedures may be directed to Georgiann Fisher, Director of Human Resources at gfisher@upsd.org.
