Facilities Request

Upper Perkiomen School District building use form instructions and submission details:

  1. Read the Use of Facilities Policy
  2. Review the Use of Facilities Administrative Regulations and complete the application as required. Any required insurance forms must be included at the time of submission.
  3. At least two weeks prior to the requested date(s), the completed application must either be emailed to ahannon@upsd.org or mailed / dropped off at the district's Education Center .
  4. If the event is reoccurring (such as basketball practice or club meetings), one building request may be submitted, however all requested dates and times must be listed.
  5. A copy of the approved/disapproved form with any charges indicated will be returned to the applicant.
  6. Please make sure that the form is complete and signed.
  7. Please call the Facilities Office at 215-541-2444 with any questions.
