PA Climate Survey Passive Parent Consent Form
Parents and Guardians,
Critical to the success and continuous improvement of the Upper Perkiomen School District is feedback from each of the key stakeholder groups within the school community. The information obtained from students, parents, teachers, and community members is fundamental to decision-making and all school improvement efforts. This year, the administration is asking for feedback related to school climate. School climate reflects the perceptions of the school’s stakeholders. School Climate is predictive of students’ ability to learn and develop in healthy ways. When school climate is positive, school connectedness is strong. Students are more ready to function optimally and focus in the classroom when they feel both psychologically and physically safe, valued and respected for their ideas and opinions.
Cognizant of the essential role that school climate plays in student success, we are asking that all students in grades 3-12 take the short ten minute survey located on the homepage of elementary school your child attends.
.Participation in this survey is completely voluntary and will cause little or no risk to any student. The survey is designed to protect each student’s privacy. Students will not put their names on the survey. Also, no student will ever have their individual responses reported. The results of this survey will be used by your school for future improvement planning and programming. We would like all students to take part in the survey, but the survey is voluntary. No action will be taken against the school, you, or any student if the student chooses not to participate.
